3. Since your commitment to sobriety 30 days ago have you examined any material, either printed or in video form that, while not pornographic, sexually stimulated you?

This second section determines client's involvement in habits that created an atmosphere ripe for sexual addiction.
1. Since your commitment to sobriety 30 days ago have you taken any illegal drugs?
2. Since your commitment to sobriety 30 days ago have you consumed alcohol to an extent that it altered your personality?
3. Since your commitment to sobriety 30 days ago have you engaged in sexual deprivation?

This third section determines client's development of habits helpful in fighting sexual addiction.
1. Since your commitment of sobriety 30 days ago have you begun and ended each day on a spiritual basis?
2. Since your commitment to sobriety 30 days ago have you at any time been accountable to another man?

This fourth section determines client's level of success or failure in fighting sexual addiction.

2. Since your commitment to sobriety 30 days ago have you at any time retreated to your "bubble" during work time?
3. Since your commitment to sobriety 30 days ago have you at anytime acted out while in your "bubble"?
